HWMG does not automatically mail federal Form 1095-B to health plan subscribers. According to the IRS, there is no longer a penalty if you do not have a full year of health care coverage, and Form 1095-B is not required for you to file your federal tax return.

If you would like to receive Form 1095-B for your own records, you may request one as follows.

  • Call HWMG’s Customer Service Center:

  • Send a request by email* to:
    CustomerService@hwmg.org​

    You must include:
    Your first and last name
    Health plan ID number
    Contact number
  • Send a request by mail to:

Within 14 days of your request, HWMG will mail Form 1095-B to you, or you may ask to receive it by secure email.
