Participating Providers

In order to be an HWMG participating provider, providers must pass:

  • HWMG’s initial credentialing process: Providers must submit their credentials and related information, which is reviewed by HWMG’s Credentialing Committee.
  • HWMG’s recredentialing process: Providers must review and update their information every three years.

Providers are expected to notify HWMG of any changes as soon as possible. Before obtaining services, please verify whether a provider is currently participating by asking the provider’s office, accessing our online directory search, or contacting us for assistance.

We make every effort to keep the information on this site current and accurate, but there are limitations because providers supply their own data to HWMG. It is important you know what information is verified and what is not.

Name. The name of the physician, hospital, or facility.

  • Limitation: The provider gives us this information.

Gender. The physician’s gender.

  • Limitation: The provider gives us this information.

Practice Location. The street address, island, and phone number of a practice.

  • Limitation: The provider gives us this information.

Hospital and Other Affiliations. Hospitals and other practices where a physician is approved to provide patient care, which may include admitting privileges.

  • Limitation: The provider gives us this information.

Specialty. The provider’s area of practice (e.g., branch of medicine, procedures, age range of patients, body systems, or types of diseases).

  • Limitation: None; HWMG verifies this information.

Non-English Languages. Languages other than English spoken at a practice.

  • Limitation: The provider gives us this information.

Status to Accept New Patients. Whether a provider is able to take on new patients.

  • Limitation: The provider gives us this information.

Board Certifications. Physician’s area of training or area in which they have passed a medical specialty examination.

  • Limitation: None; HWMG verifies this information.

Accreditations. The accredited board which has certified a hospital meets the board’s quality standards.

  • Limitation: None; HWMG verifies this information.
